Only one person has access to such database and will only extract pertinent data upon verified request from authorized government agencies after a strict and thorough approval process.

I also understand that, while UG employs all necessary precautions to safeguard my information, 100% cybersecurity cannot be guaranteed. Given this, I will use the latest version of web/mobile browsers, equipped with security features that provide anti-phishing protection, improved parental controls and tools to prevent malware and other privacy threats.

I hereby knowingly and voluntarily acknowledge and confirm that I have been duly informed of my rights under the law with respect to my personal and health information provided herein. I understand that my personal information is protected under the Data Privacy Act of 2012; and, I am required under RA 11469, Bayanihan To Heal As One Act to provide truthful information.
